First Case on Time Starts

January 26, 2022

As we continue working to increase OR efficiencies and maximize patient throughput, another key area of focus is on first case on time starts (FCOTS). First case delays in our ORs create a ripple effect for the remainder of the day, delaying subsequent cases, frustrating patients and staff, decreasing efficiency and increasing costs. To mitigate these issues, our HMSL surgical leadership team has set a goal of 80% FCOTS. If you’re scheduled for the first case of the day, please arrive at least 15 minutes prior to your scheduled start time to ensure an on time start (e.g., if your case start time is at 7:30 a.m., you must arrive no later than 7:15 a.m.).

We’re monitoring all causes of delays, not simply surgeon related delays (e.g., anesthesia, equipment, staff, etc.). FCOTS will be monitored, reviewed and reported monthly to our HMSL OR Executive Council and Medical Executive Committee. For surgeon related delays, if a trend of late first case starts is identified within a three-month period, the ability to schedule the first case of the day may be restricted.
